Timeline Of System Failures
2015-2107: Treated by Community Bridges for Major Depression Disorder
4/15/17: MCSO asked for a psych referral because Michael was hearing voices and “was just doing what he was told.”
5/16/17: Screened for SMI and diagnosed with antisocial personality disorder. SMI denied.
5/20/17: Taken to UPC(Urgent Psychiatric Center) for involuntary treatment as he was having a conversation with the voices in his head. He was diagnosed with psychosis.
5/21/17: He came back to UPC the next day- this time voluntary.
6/6/17: Admitted to Banner Behavioral and presented with worsening psychosis. Was having paranoid delusions. He was sent back to UPC involuntarily. Schizophrenic Diagnosis.
6/14/17: Back at UPC: Paranoid and irrational. Refusing medications. Again diagnosed with psychosis.
6/19/17: Booked into County.
6/28/17 MCSO documented delusions.
10/11/17: ER at Arizona General Hospital. Appeared to be laughing inappropriately and talking to himself. Diagnosed with brief psychotic disorder.
10/22/17: Police sent him back to UPC for dangers to others and speaking to voices in his head. Concerned people are following him and have been for years. He has attempted to stab people on two occasions. Believed the Mexican Mafia was after him. As to the stabbing- says he was trying to be “proactive.” Diagnosed with Psychosis NOD.
11/2/17: He arrives at MCSO for the assault booking. While at county he is seeing psych, having multiple issues, is put on Risperidone.
3/3/18: Assessed again for SMI. Again denied. Diagnosed with stimulant disorder and adult antisocial disorder.
10/26/18: ADOC: They acknowledge he is a violent inmate and has paranoia- suicide watch ordered
10/27/18-10/30/18: Put on Risperidone for 3 days.
10/28/18: Paranoid and bizarre behavior
11/8/18: Refusing meds
11/15/18 Fight with another inmate- diagnosed with no active disorder.
6/3/19 Request by Michael for psych- feeling bad, darkness coming, needs to get his cells re-cellularized
7/2/19 DOC RELEASE – Zero continuity of mental health care – Released to the streets
7/2/19 Multiple contact with law enforcement in Yuma area. Bizarre behavior with officers, at the Black Bear Diner, and O’Reilly Auto Parts
07/04/19 In custody for homicide
04/17/21 Found dead – No known cause
